‍Understanding Assessment templates

Assessment templates are designed to help you evaluate your clients' needs by using standardized forms such as GAD-7, PHQ-9, PHQ-15, PCL-5, and others. These templates streamline the process of gathering insights into your client’s condition and are an essential tool for diagnostics. Eggmed provides a library of ready-to-use templates that you can edit to suit your needs.

Assigning an assessment template to a client

1. Assigning an assessment template to a new client

When adding a new client, toggle on Send intake documents in the Add Client slideout. Click the + Add a document button, and a search bar will appear. Type the name of the assessment template you wish to send.

Note: Unlike intake templates, assessment templates cannot be set to auto-send.

2. Assigning an assessment template to an existing client

Option 1: From the client profile

1. Navigate to Client profile > Activities > Assign activities button.
2. Select Form from the dropdown menu.
3. In the slideout modal, go to the Assessment tab, select the desired template.
4. Click Assign. You can set the assignment frequency here.

Option 2: From the settings page

1. Go to settings > templates > assessment templates.
2. Click the ... button next to a template and select Assign to client.
3. In the pop-up, choose one or more clients to assign the template to, or assign it to all clients. You can also set a frequency for recurring assignments.

Creating an assessment template

Option 1: Creating an assessment template from the settings page

1. Go to settings > templates and click + add.
2. Complete the template fields. For detailed instructions, refer to this template creation article.
3. Under Save this template as, select Assessment, then click Save. The template will be categorized under assessment templates.

Option 2: Creating an assessment template from the client profile

1. Go to the client profile.
2. Navigate to activities and click assign activities.
3. Choose Form from the dropdown to open the slidout modal.
4. Under the assessment tab, click create new form to design your template directly.
